The Major Report (http://www.majorcomm.ca/en/reports/finalreport/) makes it clear there were serious failures before the bombing. That is tragic - but Air India was before 9/11 and it seems there have been improvements since then.
The real issue is the cover-up that followed the attack and which, perhaps, continued through to the Inquiry itself.
That's the real problem. The RCMP has a proud tradition -- as it should -- and the large majority of its officers are honourable men and women determined to do the right thing. Sadly, as the Ian Bush Incident, the Robert DziekaĆski Taser incident and the Royal Inland Hospital Taser Incident, to name a few, suggest, there are ongoing issues with the RCMP.
In fairness, the Prime Minister has acted quickly on the Major Report by reaching out to the families of those killed -- perhaps he will take the harder step to reform the RCMP?
(And just a note - before I get the comments about Liberal failures - the problems with the RCMP date back a long way and both Conservative and Liberal governments could have made needed reforms. That is no reason not to do now what should have been done before)

I'd say it was the politically correct atmosphere that refused to identify and accept the existence of ethnic terrorism, not just the RCMP. It's as if the mention of a radical group was an attack on all. It's also a hard thing to accept when we have MPs marching under the flags of other terrorist groups like Hamas, Hezbollah etc. I finally agree with Ujjal Dosanjh on something:
“Where we have gone wrong in this pursuit of multiculturalism is that there is no adherence to core values, the core Canadian values, which [are]: That you don’t threaten people who differ with you; you don’t go attack them personally; you don’t terrorize the populace.”
